So often, parents (& even clinicians!) can get caught up on "milestones" that a child "should have" reached by a certain age. But a quick Google search will often show really conflicting information for milestones, which can lead to further confusion or anxiety in parents.
"Milestones" are just guidelines, similar to how BMI is a guideline (but often NOT accurate and inappropriate for many people). As a parent, you know best and what feels right. Someone working with your child should be looking at the full picture and your child's specific strengths, instead of comparing to numbers that may not be representative for a multitude of reasons. Your quick Monday reminder that language therapy (AND AAC therapy) shouldn't *ONLY* focus on requesting.
While many late-talkers or AAC users are motivated by asking for something, we need to ensure we are modelling and teaching all the reasons we communicate! If we don't, we may be limiting our children's scope of communication and the connections they can build with others. Just think about how boring it would be if you could only ask for something, and not comment, tease, protest, narrate, or exclaim! Kicking off AAC Awareness month with some differences between high tech and low tech AAC!
While it is always a good idea to have low tech available as a backup, remember there is NO prerequisite for high tech (and robust!) I LOVE Stage 4 of the Natural Language Acquisition (NLA) framework for gestalt language processors, because for the first time, kids are now *self-generating* their own grammar and are no longer scripting adult language. However, this means they WILL be making mistakes as they learn, and will sound "younger" than when they were scripting adult language models. Instead of focusing on correcting errors, we want to celebrate the new grammar concepts they are gaining such as:
When we hear grammar errors, we don't need to worry about correcting! We can just validate what the child said and recast back with the correct grammar, without explicitly correcting. The most valuable lessons I have learned working with remote Indigenous communities over the past 3 years has been *unlearning* certain assumed communication expectations in order to provide culturally-sensitive SLP care.
Of course, every community I work with is different, and different cultures will have different communication styles, but these points are something that I like to consider when working with Indigenous populations, or any under-represented population. We often hear the term "strength-based" tossed around, but what does that really mean?
Traditionally, medical professions have followed a "medical model of care," which focuses on diagnosing, pathologizing, and fixing a "problem." And in many facets of healthcare, we need this (think: acute care or conditions)! More recently, developmental & rehabilitative professionals have been moving towards a "strength based model of care." This means there is LESS focus on fixing a "problem" or comparing to norms. This is especially important when looking at intervention for those who aren't neurotypical and/or interact with the world in different ways, or those with chronic conditions. However, we NEED both models, and like anything, it's about balance! You wouldn't want to show up to the ER and not have your bloodwork compared to norms; you NEED that diagnostic information. Similarly, many autistic children need the medical model for diagnosis in order to receive funding. We are SO excited to share that we have partnered with Options Northwest to utilize their amazing Rheal's Room (a Snoezelen room located in their centre).
Rheal's family graciously funded a significant amount towards this room as a tribute to his life, for people of all ages in our community to use. Halle will be able to see current clients at the centre using this room, for no extra fee. We are hoping to have more HAEPI staff trained soon, to see clients here as well! Let's talk pronoun reversal in gestalt language processors.
As a parent, you may have wondered why your child always seems to confuse "I" and "you"? As an SLP, you maybe have wondered, how do we target this? When we understand that our GLPs are picking up verbatim utterances from adult models that they have attached meaning to (as their gestalts), it makes a lot of sense! When we honour and understand these utterances without trying to change them, and instead change how WE model language, that's when we will see a shift happen. We can expect NOT to see kids began to tease apart these pronouns on their own until stages 3-4, so we don't want to rush it. Instead, focus on modelling from the child's perspective or a joint perspective (e.g., "we are ____"; "let's go _____"). Have questions about pronoun reversal in GLPs? We have had such a great response to our baby/toddler check-in sessions, and are thrilled to share that we will have 3 SLPs (Halle, Megan, Linsey) able to provide these services over the summer months of July & August.
Here are the answers to a few FAQ: Where do sessions take place? We come to you! We can work with you at your home, daycare, or community setting (park, playground, etc.) within Thunder Bay. Most parents LOVE that we can see you at home, since that's one less transition for you and your little one (plus not having to find childcare for siblings is a bonus!), and we can provide strategies to try using the things you already have in your home! What is the frequency of sessions? This is flexible, and we will work with you and your child to determine the best frequency. It may be one time, biweekly, monthly, or every couple of months. Services can be paused or discontinued at any time. Is this service eligible for reimbursement through private insurance/benefits? Absolutely! Many insurance plans cover speech language pathology services. We recommend checking with your benefits provider first to clarify what coverage is available by your plan . Have any other questions?
